In traditional Chinese medicine, kidney yang is the source of all heat and vitality in the body. It is the fire of the Ming Men - the gate of life - which sits between the two kidneys and supplies the entire organism with warmth. The uterus - referred to as Bao Gong (奇恒之腑) in TCM - is an extraordinary organ that is warmed and nourished directly by Kidney Yang. It is connected to the kidney via the Bao Mai (Uterine Vessels) and to the heart via the Bao Luo (Uterine Reticular Vessels). When Kidney Yang is strong, Qi and blood flow freely through the uterus, menstruation is regular and painless, and fertility is intact.

However, if Kidney Yang is weakened, the uterus loses its warming supply. Cold penetrates the lower warmer and „freezes“ the free flow of Qi and blood. TCM says: „Cold causes clotting“ - the blood becomes viscous, the flow of Qi stops and the uterus becomes cold and stiff. The result: pain, irregular menstruation and possibly difficulties getting pregnant.

The result of a kidney yang deficiency with coldness in the uterus: The lower heater is no longer sufficiently warmed. The Yellow Emperor: „Qi Bo, in my realm many women complain of a cold abdomen, severe pain during menstruation and difficulty conceiving children. Their hands and feet are icy, they freeze even in summer and feel tired and listless. What is the cause of this suffering?“

Qi Bo: „Your Majesty, these women suffer from a deficiency of Kidney Yang combined with Coldness in the Uterus - Shen Yang Xu with Gong Han. Kidney Yang is the fire of the Ming Men - the Gate of Life - which sits between the two kidneys. This fire is the source of all heat in the body. It warms the Lower Warmer, drives digestion, nourishes the bones and - most importantly - warms the Uterus, the Bao Gong. When this fire goes out, cold penetrates the lower warmer. The uterus becomes cold, the blood coagulates and the flow of Qi stops. The result: pain that improves with warmth, irregular bleeding and infertility.“

Dr. Weber: „That's a very precise description! In Western medicine, we often see a combination of hormonal imbalance and reduced blood flow in the pelvic area in these patients. The ‚inner cold‘ of TCM correlates surprisingly well with measurable parameters: a low basal body temperature below 36.5 °C, reduced progesterone levels in the luteal phase and reduced blood flow to the uterus, which we can detect using Doppler ultrasound. Progesterone is also called the ‚warming hormone‘ because it raises the body temperature by 0.3-0.5 °C in the second half of the cycle - exactly the ‚warmth‘ that the uterus needs.“

The Yellow Emperor: „The fire of the gate of life - how does it start and why does it go out?“

Qi Bo: „Your Majesty, the Ming Men Fire is part of the Kidney Essence (Jing), which we partly inherit from our parents and partly build up from food. It can go out for several reasons: Firstly, through constitutional weakness - some women are born with a weaker Ming Men fire. Secondly, through chronic overwork and exhaustion - they deplete the Yang faster than it can be replenished. Thirdly, through too many births or miscarriages - they deplete Kidney Essence. Fourthly, through excessive consumption of cold food and drink - the inner cold extinguishes the fire. And fifthly, through emotional stress - chronic anxiety directly weakens the kidneys.“

Dr. Weber: „Fascinating! This is completely in line with our findings! The constitutional weakness corresponds to genetic variations in hormone production. Chronic stress increases cortisol and suppresses the hypothalamic-pituitary-gonadal axis, which means less estrogen and progesterone. Frequent pregnancies and miscarriages actually put a strain on the hormonal system and the adrenal glands. And a diet with lots of cold foods? It has been shown to reduce blood flow in the abdomen and can slow down the metabolism. Even anxiety is right: Chronic anxiety permanently activates the sympathetic nervous system and reduces blood flow to internal organs, including the uterus.“

The Yellow Emperor: „You talked about the cold in the uterus. What exactly happens there?“

Qi Bo: „Your Majesty, if the Kidney Yang no longer sufficiently nourishes the Ming Men fire, the uterus loses its source of heat. In TCM, cold has a very specific effect: it causes the blood to clot and contract. The blood becomes viscous and dark and forms clots. The vessels constrict, the flow of Qi stagnates. The uterus cramps - hence the severe pain before and during menstruation, which is immediately relieved by a hot water bottle. The lining of the uterus cannot build up properly because it does not receive enough warmth and blood. And without a warm nest with a good blood supply, an embryo cannot implant - hence the fertility problems.“

Dr. Weber: „This is pathophysiologically very convincing! The ‚coagulation by cold‘ corresponds to increased blood viscosity and a tendency to microthrombosis in the uterus. Vasoconstriction - the contraction of blood vessels - is a known mechanism in dysmenorrhea: prostaglandins and endothelin cause vasospasm in the uterine wall, which leads to ischemia and pain. Heat has a vasodilating and antispasmodic effect - that's why the hot water bottle helps so well! And as far as fertility is concerned: a warm endometrium with a good blood supply and sufficient thickness is actually a basic prerequisite for implantation. Studies show that an endometrial thickness of less than 7 mm significantly reduces the implantation rate.“

The Yellow Emperor: „What signs on the body reveal a kidney yang deficiency with coldness in the uterus?“

Qi Bo: „The signs are very characteristic, Your Majesty. The tongue is pale, swollen and moist - the yang can no longer transform the fluids. The coating is thick and white, especially in the posterior area - the kidney area. The pulse is deep, weak and slow - Chen, Ruo and Chi Mai. Especially at the Chi position, which corresponds to the Kidney, the pulse is barely palpable. The hands and feet are cold, the abdomen feels cool. The lower back hurts - this is where the kidneys are located. And the discharge is watery and clear - no heat to transform the fluids.“

Dr. Weber: „The pale, swollen tongue corresponds to reduced peripheral blood flow and possible fluid retention due to slowed metabolism - typical in hypothyroidism! The slow pulse is a sign of low blood pressure and reduced sympathetic tone. The cold extremities are a classic sign of peripheral vasoconstriction - the body centralizes the blood because not enough heat is being produced. And the watery discharge? It can indicate a low oestrogen level or impaired mucosal maturation. Here, TCM diagnostics detects systemically what we measure with individual laboratory values!“

The Yellow Emperor: „Now let's talk about the treatment. How can we rekindle the fire?“

Qi Bo: „Your Majesty, the treatment principle is: Wen Shen Zhu Yang, Nuan Gong San Han - warming the kidney yang, warming the uterus and dispersing the cold. This is done in several ways. First: Moxibustion - burning mugwort (Ai Ye) over certain acupuncture points is the most important method! It brings deep, penetrating heat directly into the lower warmer. Particularly effective on Conception Vessel 4 (Guan Yuan), Conception Vessel 6 (Qi Hai) and Kidney 3 (Tai Xi). Secondly, warming herbs - above all You Gui Wan, the ‚pill for the right side‘, which directly strengthens Kidney Yang. Also Wen Jing Tang, which warms the menstruation. Thirdly, a warming diet - lamb, ginger, cinnamon, walnuts and fennel warm the Lower Warmer. Fourth: Acupressure on Ni 03, Ni 07 and KG 04 - they strengthen the kidney and warm the uterus.“

Dr. Weber: „Moxibustion is particularly interesting from a scientific point of view! Studies show that it can increase local blood circulation by up to 40%. For dysmenorrhea, it is more effective than ibuprofen in several randomized controlled trials - with fewer side effects! The heat stimulates heat receptors (TRPV channels) which trigger a cascade of vasodilatory and analgesic responses. Ai Ye (Artemisia argyi), which is used for moxibustion, releases essential oils when burned, which have an anti-inflammatory effect. As for herbs: Du Zhong (Eucommia) and Rou Gui (Cinnamon) from You Gui Wan have been shown in studies to have positive effects on thyroid function and the adrenal glands. And ginger is well researched as a thermogenesis booster - it has been shown to increase metabolic rate and body temperature.“

The Yellow Emperor: „What are the most important herbal formulas?“

Qi Bo: „Your Majesty, for Kidney-Yang deficiency with coldness in the uterus, two main prescriptions can be considered. You Gui Wan - the pill for the right side - strengthens Kidney-Yang at its root. It contains: Shu Di Huang to nourish the Essence, Shan Yao and Shan Zhu Yu to strengthen the Kidney, Du Zhong and Rou Gui to warm the Yang, and Gou Qi Zi and Tu Si Zi to strengthen fertility. For severe menstrual pain caused by cold, Wen Jing Tang - the decoction for warming the menses - is suitable as it simultaneously warms and moves the blood. The combination of both principles - strengthening Yang AND dispelling cold - is the key to success.“

Dr. Weber: „You Gui Wan is one of the best researched Yang tonifying formulas! Animal studies show that it can increase levels of luteinizing hormone (LH) and progesterone, which directly improves corpus luteum function. Tu Si Zi (Cuscuta) contains flavonoids, which have an estrogen-like effect and support follicle maturation. Rou Gui (cinnamon) has been shown to improve insulin sensitivity, which plays a role in many fertility problems. And Wen Jing Tang? A Japanese study showed that it could significantly improve pregnancy rates in women with unexplained infertility - presumably by improving blood flow and mucosal quality.“

The Yellow Emperor: „Qi Bo, how long will it take for the fire of the Gate of Life to burn again?“

Qi Bo: „Majesty, building the kidney yang takes patience - it is a profound process that affects the root of the body. After 2-3 weeks of consistent practice, most people notice: less cold feeling in the abdomen, warmer hands and feet, better digestion. After 6-8 weeks the improvements become more noticeable: less menstrual pain, more regular cycle, more energy and drive. After 3-6 months, the kidney yang can be noticeably strengthened - the uterus feels warm, menstruation normalizes and the chances of conceiving improve significantly. The duration depends on how long the cold has been present and how consistently the treatment is carried out.“

Dr. Weber: „This is very much in line with our clinical experience! With hypothyroidism, we see the first improvements in symptoms after 2-4 weeks of therapy, but complete normalization often takes 3-6 months. The improvement in uterine blood flow can be demonstrated by Doppler ultrasound after just a few weeks of moxibustion. And for women who wish to have children, we usually recommend at least 3 cycles - i.e. about 3 months - of TCM treatment before assessing the results. The TCM recommendation of 3-6 months is therefore very realistic!“

The Yellow Emperor: „What must be avoided at all costs in the case of kidney yang deficiency with coldness in the uterus?“

Qi Bo: „Your Majesty, the most important thing is: firstly, no cold food or drinks - they further extinguish the already weak yang fire. No ice, no smoothies, no cold water. Secondly, always keep the abdomen and lower back warm - no belly-baring clothes, no sitting on cold stones. Third, no excessive physical exhaustion - it depletes yang. Fourth, avoid cold environments - swimming in cold water, long stays in air-conditioned rooms. Fifth, do not cultivate anxiety and fear - anxiety directly weakens the kidney. And sixth, pay special attention to warmth during menstruation - this is when the uterus is most sensitive to cold.“

Dr. Weber: „Scientifically absolutely comprehensible! Cold food and drinks cause a reflex vasoconstriction in the abdomen - blood flow to the internal organs is reduced. Wearing warm clothing over the lower abdomen maintains the local temperature in the pelvic area, which is important for mucosal maturation. Intense physical exertion increases cortisol and can suppress ovulation - many competitive athletes have cycle disorders. Exposure to cold activates the sympathetic nervous system and reduces pelvic blood flow. And chronic stress and anxiety disrupt the HPG axis and can directly affect the cycle. All TCM recommendations are endocrinologically and physiologically based!“

Who are the Yellow Emperor and Qi Bo?

The Yellow Emperor (Huangdi) is said to have lived around 2600 BC and is considered the mythical founder of Chinese culture and medicine. Qi Bo was his most important advisor and personal physician. Their discussions about health, illness, and human nature were recorded in the most important fundamental work of TCM.

The book: Huangdi Neijing

The Huangdi Neijing, also known as The Yellow Emperor's Classic of Internal Medicine, was probably written between 200 BC and 200 AD. It is the oldest surviving work on Chinese medicine and consists of dialogues between the Yellow Emperor and his advisors, mainly Qi Bo.

This work is to TCM what the Bible is to Christianity: the absolute foundation. All the important concepts - yin and yang, the 5 elements, qi, meridians and acupuncture points - are systematically described here for the first time.

All exercises work particularly well if you practise them between 5 and 7 p.m. - this is the peak of the kidney in the organ clock. The morning is also a good time to activate the yang.

QI GONG:

When and how often?

In the morning or late afternoon (ideal):

  • All 3 exercises as a sequence: approx. 15–20 min.
  • Or just 1–2 exercises: approx. 5–10 min.
  • Preferably between 5 and 7 p.m. (kidney time)

In between:

  • Individual exercises if required (e.g. if you feel cold)
  • „Warm the center“ for rapid heat supply

Regularity is more important than duration:

  • It is better to exercise for 10 minutes every day than for 60 minutes once a week.
  • After 2–3 weeks, you will notice the first changes.
  • After 2-3 months of regular practice: significantly more warmth and energy

Pro tip: Warming, gentle movements are ideal for kidney yang deficiency. Avoid intensive sport and heavy sweating - this consumes Yang and Qi. Qi gong with a focus on the lower warmer is the perfect method to strengthen the yang.

Both exercises activate the kidney meridian, which runs along the inside of the legs and is connected to the kidneys. The kidney stores essence and is the source of yang - by activating this meridian, you support both heat production and fertility.

Particularly effective: Hold each position for 3-5 breaths, visualizing warm, golden energy flowing from the kidneys into the uterus to dispel the cold.

The kidney yang belongs to the element Water and needs warm to hot, invigorating food, to nourish the fire of the gate of life. Everything that warms from within strengthens your yang.

What you eat, HOW you eat and WHEN you eat - all of this influences your yang and your inner warmth.

  • Lamb (the strongest Yang tonic!)
  • Shrimps and crabs - warm the kidneys
  • Walnuts - nourish the kidney yang and the essence

RECIPE: LAMB SOUP WITH GINGER AND WALNUTS

Ingredients:

For 4 portions

  • 500 g lamb (shoulder)
  • 3 cm fresh ginger (sliced)
  • 50 g walnuts
  • 1 sweet potato (diced)
  • 1 leek stalk
  • 1 cinnamon stick
  • 5 black peppercorns
  • 1.5 L water
  • salt to taste
  • Optional: 2 slices of dang gui (angelica root)
Preparation:
  1. Wash the lamb and cut into pieces
  2. Add cold water and heat slowly
  3. Skim off the foam, add the ginger and spices
  4. Simmer over a low heat for 1.5 hours

  • 1 cinnamon stick or 1 teaspoon of cinnamon powder
  • 3 red dates (pitted)
  • 700 ml water
  • 1 tbsp honey
  • Optional: 3 slices of fresh ginger
Preparation:
  1. Wash the rice and add water
  2. Add the cinnamon stick and dates
  3. Simmer over a low heat for 45-60 minutes (stir regularly)
  4. Dry roast the black sesame seeds in a pan

It warms the entire body, drives the life force and nourishes the uterus with its flame. If this fire goes out, the cold takes hold of the lower warmer and life comes to a standstill.

More tips for your everyday life

In addition to traditional TCM treatment methods there are many small everyday habits that can strengthen your kidney yang and drive the cold out of your uterus. In TCM, the kidney is the source of life force - with the right habits, you can effectively rekindle its fire.

Heat and cold protection

Keeping the abdomen warm

Heat is the most important medicine for cold in the uterus:

  • Wear a kidney warmer or haramaki - protects the kidneys and uterus
  • Always keep your stomach and lower back covered
  • Never sit on cold surfaces (stone, metal) without a base
  • Hot water bottle or cherry stone cushion on the abdomen if you feel cold
  • Wear warm underwear and socks in winter
Warm foot bath

The kidney meridians start at the soles of the feet:

  • 15-20 min. warm foot bath (38-42 °C) in the evening
  • With fresh ginger (3-5 slices) and mugwort leaves
  • The kidney meridians start at the soles of the feet - heat rises from there
  • Promotes blood circulation in the entire pelvic area
  • Put on warm socks immediately after the footbath
Moxibustion at home

Moxa heat dispels the cold from the depths:

  • Moxa cigars are easy to use yourself
  • KG 04 (Guan Yuan) - the most important moxibustion area for cold in the uterus
  • Slowly circle for 10-15 minutes, 3-5 cm away from the skin
  • 3–5 times per week for best results
  • Do NOT use during menstruation in case of heavy bleeding

Sleep and rest

Sufficient sleep

The kidney yang regenerates during sleep:

  • 7–8 hours of sleep per night is ideal
  • Kidney yang deficiency may also require more sleep
  • Kidney energy regenerates between 5 and 7 pm - a good time for rest
  • Go to bed before 11 pm - the kidney yin nourishes itself in the late hours
  • Warm feet when falling asleep - socks or hot water bottle on your feet
Avoid revision

Exhaustion depletes kidney yang:

  • Chronic exhaustion depletes the kidney yang
  • Plan regular breaks - every 90 minutes.
  • Don't push yourself when you're tired
  • In the case of kidney yang deficiency: manage energy, do not overstrain
  • Plan sufficient vacation and relaxation

Gentle movement - generate heat, don't consume it

The right intensity

Gentle movement generates heat without consuming Yang:

  • Gentle, regular exercise is ideal - it generates heat without consuming Yang
  • Walks in the fresh air (dress warmly!)
  • Qi Gong and Tai Chi - warm up without exhausting yourself
  • Gentle yoga in a warm environment
  • Dancing - activates blood circulation in the pelvic area
What you should avoid

These activities consume Yang and allow cold to penetrate:

  • Swimming in cold water - cold penetrates through the skin
  • Intensive endurance sports (marathon, HIIT) - consume Yang
  • Heavy sweating - depletes the body fluids and Yang
  • Exercise when tired or feeling cold
  • Long stays in air-conditioned rooms

For women: Menstruation and the desire to have children

During menstruation

Take extra care of yourself and keep warm:

  • Extra warmth for the abdomen - a hot water bottle is a must!
  • Hot food and drinks - no raw food, no ice cream
  • No intensive sports units
  • No cold water - not even for washing your hands
  • Pause moxibustion in case of heavy bleeding
If you want to have children

Cold in the uterus is one of the most common TCM causes of infertility:

  • 3-6 months TCM preparation is ideal
  • Measuring basal body temperature - shows the success of the treatment
  • Professional TCM advice for an individual herbal plan
  • Stress reduction is crucial - anxiety weakens the kidneys
  • Be patient - the body needs time to warm up

Important to know

Strengthening the kidney yang takes time and patience. Warming up the body is like lighting a fire - it happens step by step. Pay attention to your body's signals: cold feet, shivering and tiredness show that you need more warmth and rest. With consistent care, you will feel the warmth and vitality returning to your body.

When to see a doctor? If you do not menstruate for more than 3 months, have severe pain, are unable to conceive for more than 12 months or have other worrying symptoms, you should consult a doctor and have your thyroid and hormone status checked. TCM and Western medicine complement each other perfectly here.

For optimal treatment progress

TCM is not a quick fix for symptoms., but rather a holistic healing process. You will achieve the best results if you consistently integrate the treatment into your daily routine and give your body time to regenerate.

Integration into everyday life

Start with ONE step

Don't try to change everything at once! That's overwhelming and leads to
usually leads to you giving up. Instead:

  • Week 1-2: Change your diet - integrate warming foods, avoid cold foods
  • Week 3-4: Acupressure Ni 03 and KG 04 daily, moxibustion if possible
  • Week 5-6: Qi Gong „Warming the center“ daily 10 min.
  • Week 7-8: Supplement TCM herbal mixture
  • From week 9: Add further methods as required
Realistic expectations

After 2–3 weeks: First improvements noticeable

  • Feeling less cold in the abdomen
  • Hands and feet get warmer
  • Improved digestion

After 6–8 weeks: Significant improvements

  • Significantly less menstrual pain
  • More regular cycle

  • Kidney yang noticeably strengthened
  • Uterus feels warm and supplied with blood

TCM vs. Western medicine

TCM and Western medicine are not mutually exclusive—they complement each other!

When Western medicine is important:

  • In acute emergencies
  • In case of structural damage
  • In case of infections
  • For accurate diagnoses (laboratory, imaging)

Where TCM has its strengths:

  • In case of functional disorders
  • For chronic complaints
  • For prevention
  • For holistic healing

Ideal: Combine both systems! Use Western medicine for
Acute treatment and accurate diagnosis, TCM for lasting healing
and strengthening the body's self-healing powers.

Important: Never stop taking prescribed medication without consulting your doctor first!
Talk to your doctor when you start TCM treatment.

Syndrome differentiation according to TCM:

Tongue: Pale, swollen, moist, thick white coating, especially in the posterior region

Pulse: Deep (Chen Mai), weak (Ruo Mai), slow (Chi Mai), especially at the Chi position (Kidney)

Pathomechanism:
The kidney yang is exhausted and can no longer fulfill its warming function. The Ming Men fire (life gate fire) goes out, the lower warmer is penetrated by cold. The uterus (Bao Gong) is no longer sufficiently warmed, which leads to stagnation of cold. The Cold in the Uterus blocks the free flow of Qi and Blood in the Lower Heater, which can lead to dysmenorrhea, amenorrhea or infertility. There is often a simultaneous deficiency of Spleen Yang, as Kidney Yang forms the basis for Spleen Yang.

Treatment principle:
Wen Shen Zhu Yang, Nuan Gong San Han (温肾助阳,暖宫散寒) - Warm and support the Kidney Yang, warm the uterus and dispel the cold

Classic recipe:
You Gui Wan (pill for the right side) for basic kidney yang deficiency
Wen Jing Tang (decoction to warm the menses) for severe cold stagnation in the uterus

Important modifications depending on accompanying symptoms:

  • For severe dysmenorrhea: + Ai Ye (Artemisiae Argyi Folium), Xiang Fu (Cyperi Rhizoma) - warms the uterus and moves the Qi
  • For infertility: + Tu Si Zi (Cuscutae Semen), Xu Duan (Dipsaci Radix), Ba Ji Tian (Morindae Officinalis Radix) - strengthens the essence and fertility
  • In case of pronounced cold with watery discharge: + Cang Zhu (Atractylodis Rhizoma), Fu Ling (Poria) - dries moisture and warms
  • For accompanying Spleen Yang deficiency: + Bai Zhu (Atractylodis Macrocephalae Rhizoma), Gan Jiang (Zingiberis Rhizoma) - warms the spleen and strengthens transformation
  • For yang deficiency with blood stasis: + Tao Ren (Persicae Semen), Hong Hua (Carthami Flos), Yi Mu Cao (Leonuri Herba) - moves the blood and releases stasis
